Job Description

  • Hours: Monday: 7:30 - 6:30 (extended hours rotated by providers on staff), Tuesday through Friday: 8 - 5. Each provider gets a half-day per week for admin time

  • Additional Information: Seeking a compassionate and patient-centered Primary Care Physician. This position reports directly to the Chief Medical Officer and plays a vital role in delivering high-quality, comprehensive care within our FQHC.

    The PCP will serve as both an independent and collaborative member of the healthcare team, providing full-scope primary care services while supporting our mission to improve access to care for the communities we serve.

    Key Responsibilities
    - Assess, diagnose, plan, and provide comprehensive primary care services to patients across the lifespan
    - Deliver care in a courteous, efficient, and professional manner consistent with FQHC standards
    - Provide clinical guidance and supervision to medical assistants, nursing staff, and medical students as applicable

    Qualifications
    - Eligible for and able to obtain medical licensure in the state of Indiana
    - BE/BC in Family Medicine
    - Current CPR certification
    - Ability to meet credentialing requirements of affiliated hospitals
