Additional Information: Licensure: Current, unencumbered license and registration as a Licensed Practical Nurse
(LPN) in the State of New York.
Education /Training: Graduation from an accredited Licensed Practical Nursing program or GED equivalent approved vocational nursing school. High school diploma or GED Equivalent.
The LPN plays an essential role within Heritage Health and Housing's ambulatory clinical practice, providing direct patient care and clinical support under the supervision of a Registered Nurse (RN), Physician, or Nurse Practitioner.
You will support efficient clinical operations and helps ensure patients receive compassionate, high-quality care. Responsibilities include coordinating clinical flow, preparing patients for examinations, administering medications and immunizations, performing diagnostic screenings, and providing patient education tailored to diverse patient populations.
