The Licensed Practical Nurse (LPN) plays an essential role within Heritage Health and Housing's ambulatory clinical practice, providing direct patient care and clinical support under the supervision of a Registered Nurse (RN), Physician, or Nurse Practitioner.
As a member of the patient-centered medical home team, the LPN supports efficient clinical operations and helps ensure patients receive compassionate, high-quality care. Responsibilities include coordinating clinical flow, preparing patients for examinations, administering medications and immunizations, performing diagnostic screenings, and providing patient education tailored to diverse patient populations.
