For over three decades, the Family Practice and Counseling Services Network (FPCSN) has stood as a trusted leader in community-based, nurse-managed, and patient-centered care across Philadelphia. Founded with a vision of empowering communities through wellness, FPCSN provides comprehensive primary care, behavioral health, dental, social services, and supportive programs to individuals and families, regardless of the ability to pay, background, or circumstance.
FPCSN’s service area spans across Philadelphia and surrounding counties via its 6 sites: Abbottsford Falls Health Center (Northwest Philadelphia), The Stephen and Sandra Sheller 11th Street Family Health Services Health Center (North Philadelphia), Health Annex (Southwest Philadelphia), and 6 school-based health centers. Serving over 12,000 individuals across the lifespan, our guiding principle is simple yet powerful: “quality health care is a right, not a privilege.” Every day, our dedicated team of clinicians, social service professionals, and support staff live out that belief, delivering accessible, compassionate care that meets people where they are.
We are entering an exciting new chapter in our organization’s history as we move forward as a stand-alone Community Health Center (CHC), defined by growth, collaboration, and renewal. Guided by our Strategic Plan, FPCSN is focused on becoming a more accessible, efficient, and accountable organization, working to ensure that every patient, partner, and team member experiences the highest standard of excellence in both care and connection.
