Fifty years ago, a group of volunteers saw the need to provide healthcare to underserved members of the community. Forty years ago, that volunteer effort became a non-profit organization, operating out of a mobile trailer.
Today, we are a growing network of neighborhood health centers serving over 100,000 patients in San Joaquin, Solano and Yolo Counties, making us one of the area’s largest safety net providers. We offer integrated medical, dental, behavioral health, and supportive services, all using a model that puts the patient at the center of the care team.
