ChapCare has been putting patients first since 1998. For 24 years ChapCare has served the communities of the San Gabriel Valley, providing quality health care for men, women, and children of all ages. Since 2013, ChapCare has been recognized as a Patient-Centered Medical Home (PCMH). A PCMH is a team-based model of care led by a physician who provides continuous and coordinated care throughout a patient’s lifetime to maximize health outcomes.
ChapCare provides comprehensive primary healthcare services including medical, dental, and behavioral health services to almost 16,500 patients per year. The organization utilizes state-of-the-art equipment such as Electronic Health Records to ensure that our patients receive the highest quality of care.
