Compensation:
- Salary is competitive and negotiable
- $1500 sign-on bonus
- Quarterly bonus after 6 months based on quality metrics
- $100 stipend for licensure annually

DETAILS:
- Identifies triages and manages patients with medical and behavioral health problems within the primary care setting
- Works with the primary care team to treat and manage patients with chronic emotional and/or health problems efficiently and effectively
- Teaches patients, families, and staff about care, prevention, and treatment enhancement techniques.
Qualifications:
- Master's degree in Social Work or related field REQUIRED
- Bilingual (Spanish-English) REQUIRED
- Current state licensure as an LCSW, LPC, LMFT REQUIRED
