Unicare Community Health Center is a Federally Qualified Health Center with clinics located in Ontario, San Bernardino, Colton, Fontana, Pomona, and East Los Angeles. UCHC was founded in 2013 by a group of community leaders, business associates and health care professionals in response to financial challenges faced by families in Eastern Los Angeles County and Western San Bernardino County communities; which impeded their ability to afford essential health care services and other supports.
