In 2003, PhyNet Inc., a Texas nonprofit healthcare organization, was established as a physician’s network of primary care, pediatric care, internal medicine, and occupational medicine health clinics. Operating in eight East Texas communities and one in San Antonio, Texas, our physicians and advanced nurse practitioners provide quality primary care and coordinated care services. Our passion is total patient care and wellness.

Positioned to Transform Healthcare

Since the Affordable Care Act (ACA) was passed in 2012, healthcare has turned its focus to health management and monitoring as an answer to sustainability. The Centers for Medicare and Medicaid Services (CMS) is spearheading “population health,” and for good reason. Population health is composed of communities of children, adults, and senior citizens of all ethnicities. Until population health became a focus – and even now – healthcare systems treated patients all the same: When individuals got sick, they were provided with a medical assessment and medications, and then went unmonitored until a future health problem developed.

CareTeams are Health Advocates

When patients enroll in the PhyNet CCM (Chronic Care Management) program, we provide clinical evaluations to diagnose and identify “at-risk diseases.” The data is powerful knowledge our CareTeams use to customize care plans designed for prevention. Early detection is the best prevention. Care plans include wellness visits, check-ins by phone, education and coaching for a more proactive approach to achieving better outcomes. The CCM program is designed for senior adults, as their need to stay more health-conscious is especially important.
