At Summit Medical Consultants, our mission is to provide patients and their families with a unique resource and advocate throughout the Skilled Nursing, Acute Rehab, and Long Term Care stay. We coordinate with specialists and surgeons, Physical, Occupational, and Speech Therapies, Case Management, and Psychology to create a care plan with our patient's specific needs in mind. We are here to manage our patient's care and be a resource to help them navigate this unfamiliar journey.
