About Sulzbacher Center for the Homeless

Sulzbacher was founded in 1995 on the premise that homeless persons need and deserve more than just a meal and a bed to end their homelessness. Sulzbacher provides a continuum of care approach, addressing all aspects of homelessness including housing, income, and health care. Over the more than two decades since the agency opened, the services have grown to include primary health, behavioral health, dental care, and optometry for all ages, plus permanent housing, job placement, and early learning. Sulzbacher’s federally qualified health center has grown to four clinics – two adult and two pediatric and focuses not only on homeless individuals but also the under- and uninsured individuals.

As a non-profit Federally Qualified Health Center, our focus is meeting the medical, dental, and mental health needs of the insured, non-insured, under-insured and homeless people in our community.  We provide accessible, cost-effective, high quality, comprehensive healthcare in our Northeast Florida community. Sulzbacher Health Center provides integrated patient care services in primary care, behavioral health, dental, and optometry.
