  • Additional Information: Are you a compassionate physician with a passion for geriatrics? Join our team as a PACE Primary Care Physician and help older adults live independently while receiving high-quality, all-inclusive care.

    What You'll Do
    - Serve as primary care physician for PACE participants (55+)
    - Provide preventative, acute, and chronic care with a focus on aging with dignity
    - Collaborate with the Interdisciplinary team to coordinate holistic care plans. The interdisciplinary team includes on-site social work, physical and occupational therapy, a registered dietician, a day center coordinator, home care, transportation, and more.
    - Build trusted relationships with participants and their families

    Requirements:
    - MD/DO with interest in geriatrics
    - BE/BC in IM, FM, or Geriatrics
    - Active CA license + DEA
    - Meets 42 CFR Part 410.20
    - Minimum 1 year experience with frail/elderly population
