Hours: Monday - Thursday 8:00 am to 6:00 pm, Friday 8:00 am to 12:00 pm

Compensation:
- $200,000 Base Salary
- Productivity Bonus - $50.00 per patient encounter will be paid after 5,000 encounters in a 12-month time span beginning at the effective date
