- Today, SBNC consists of 4 medical clinics, 2 dental clinics, an integrated care clinic, and a Bridge Clinic.
- SBNC is an FTCA Deemed Facility.
- We are a critical health safety net for Santa Barbara County and provide care to over 22,000 unduplicated patients a year. That is 1 out of 10 people we serve in our community.
- The Care Management position provides nurse triage services and health advice for SBNC patients in person, over the phone, and via the patient portal.
- In this role, the RN will alternate working the call center and care management at different clinics as needed.
Qualifications:
- Associate or BA/BS Degree
- 5 plus years of direct patient case experience; community health and/or triage experience required
- Current CA RN License in good standing required
- Current AHA CPR Certification
- Spanish bilingual strongly preferred but not required
