NTACHC provides health services based on the Patient-Centered Medical Home (PCMH) model, which puts patients at the forefront of team-based care. At NTACHC, patients receive the highest quality comprehensive primary care medical services including pediatric care, well-woman care, chronic disease management, and more. NTACHC collaborates with other community partners to coordinate referrals for specialty services.
Northside Community Health Center formerly known as Albert Galvan Health Clinic has been renovated to accommodate serving more patients and extend its comprehensive primary care services and behavioral care with 16 exam rooms, diabetes management, and in-house lab.
