Purpose of Position:
- Provide the administrative oversight for clinical services within the Primary Care and Specialty Care Clinics
- Oversee the coordination of case management in support of the Patient Centered Medical Home (PCMH)
- Provide patient and family-focused care
- Perform leadership functions including selection, training, performance management, and corrective actions
- Review and revise policies and procedures coordinating with the Chief Nursing Officer to monitor the nursing care
- Coordinate with other disciplines to integrate treatment programs
- Monitor the credentialing of all department staff
- Oversee networking between primary care staff and case managers
Qualifications:
- Associates required, Bachelor's preferred in Nursing, Medical or related field
- 5 years working as an RN with case management experience or Physician or PA-C/NP
- 3 years supervisory experience
