  DETAILS:
  - Reaches out and engages clients with appropriate concern for their behavioral health care needs
  - Coordinates medical care with PCP for chronic and acute non-behavioral health conditions
  - Advises and counsels clients about their physical and mental health; coordinates care with case management and health education as clinically indicated
  - Assists with the identification, counseling, and care coordination, with case management and health education, of mental health and substance abuse problems
  - Works in a non-traditional environment; e.g., the non-clinic facility where homeless people congregate
  - Provider will be servicing the men who live in the shelter initially, and patient panel will expand as role expands
