Additional Information: Join our mission-driven FQHC as a key member of an interdisciplinary healthcare team delivering whole-person care. In this role, you will coordinate and provide mental health services that support patients' emotional and psychological well-being alongside their medical care.
What You'll Do
- Conduct baseline mental health assessments for patients entering care
- Provide brief counseling, crisis intervention, and community referrals
- Identify and refer patients for psychiatric evaluation and treatment when appropriate
- Collaborate closely with medical providers to deliver integrated care
Qualifications
- Master's Degree in Social Work and an active CA LCSW license, or the ability to obtain reciprocity within 90 days
- 2+ years of experience providing direct patient services in medical, clinical, or social services settings
- Experience with adults and/or adolescents in individual, group, or family therapy
Schedule
- Tuesday evenings and Saturday half-day required
