Compensation: Base salary plus industry-leading bonus plan
Additional Information: This is a hybrid position that requires working both remotely and onsite.
Qualifications and Skills Needed:
- Bachelor's or master's degree in social work
- Valid and current social worker license
- Minimum of one (1) year of clinical social work experience
- Excellent listening and verbal communication skills
- Able to make psychosocial assessments and develop and implement care plans.
- Able to provide after-hours service as needed
- Familiarity with ECW or EMRs
- Good organizational skills
- Multilingual is an advantage
- Knowledgeable in HIPAA
Duties and Responsibilities:
- Evaluate patients (existing, new and prospective) based on their needs, limitations and desires
- Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager
- Build rapport with patients and their families
- Coordinate care with other providers
- Develop care plans for patients
- Provide quick and timely resolution to patient concerns
