Additional Information: The MSW will provide services to meet the social and/or emotional needs that affect the clients' ability to achieve their goals; participate in the development of patient/client comprehensive care plans; develop policies to provide social services to clients in compliance with federal, state and local regulations.
The MSW will work in our PACE department. PACE is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.
Qualifications:
- Kansas license to practice as a Licensed Master Social Worker
- 3-5 years of professional social work experience in health care or mental health preferred
- 1 year of experience working with older adults
- Must have knowledge and skills with regard to health care coordination, pain/symptom management, crisis intervention and counseling.
