About Community Health Connections, Inc.
Founded on February 25, 2002, we have 19 years of experience as the only Federally Qualified Health Center (FQHC) serving low-income, underinsured, uninsured, publicly housed, and homeless populations in 35 cities and towns in North Central Massachusetts.
What began in 2002 at one location providing primary medical care to 4,066 patients has significantly expanded in reach and scope. Services now include medical, dental, behavioral health, pharmacy, optometry, podiatry, acupuncture. In 2021 Community Health Connections’ Fitchburg, Gardner, Leominster centers, and ACTION Health Services provided over 130,000 visits to over 29,000 patients.
Community Health Connections was founded to address barriers to care experienced by the region’s populations. Its success is evident. Access to primary care was the community’s foremost healthcare concern before Community Health Connections existed.
