DETAILS:
- Provides comprehensive assessment and diagnosis of behavioral health clients
- Evaluates crisis situations and institutes appropriate interventions
- Assists the family practice team in developing care management processes such as the use of disease management techniques, case management, and patient education to improve self-management of chronic disease
QUALIFICATIONS:
- Master's Degree in Social Work or related field REQUIRED
- DE License REQUIRED
