Job Responsibilities:
- Obtains and documents health history, performs physical examinations, educates patients on preventative health measures for both children and adults
-Knowledge of DSM-IV diagnostic attributes
- Documents and maintains patient's records of care on Health Center EMR
- Develops and institutes appropriate treatment plans per protocol. Treatment will include the care of acute and chronic medical problems as well as contraceptive services
- Participates in multidisciplinary teams to improve patient care processes and outcomes
Qualifications:
-The candidate must be able to work independently and autonomously, as there will be occasions without supervision. 1 year experience REQUIRED, 2 years preferred.
- Active DE NP and DEA licenses.
- Spanish bilingual PREFERRED but not required
