As Care Management Supervisor, you'll lead a multidisciplinary team delivering coordinated, person-centered care to individuals with complex needs. Reporting to the Director of Care Management, you'll oversee daily program operations and serve as the on-site leader for Care Coordinators, Peer Specialists, and consulting Nursing and Psychiatric partners.
This hands-on role blends leadership and direct care-you'll step in as a Care Coordinator as needed, supporting an assigned caseload and modeling best practices. As the program grows, you'll help shape new processes and take on evolving responsibilities.
Qualifications:
- 4 years+ providing case/care management services to persons with disabilities or comparable experience
- LSCW, LMSW, or LMHC licensure required
