Additional Information: Administrative Supervisor role overseeing day-to-day operations of a Care Management program serving individuals with chronic medical and behavioral health needs. Responsibilities include supervising Care Coordinators and support staff, ensuring compliance and quality standards, monitoring billing/service delivery, participating in hiring and training, overseeing admissions/discharges, conducting audits and chart reviews, coordinating crisis coverage, facilitating meetings and case conferences, and supporting community outreach efforts. Position may also provide direct care coordination services for assigned cases as needed. Requires Master's degree plus NY licensure (LCSW, LMSW, LMHC, RN, or NP), 5+ years of case management experience, 3+ years supervisory experience, strong knowledge of serious mental illness and recovery-oriented care, and proficiency with EMR/documentation systems. Hybrid-style role with approximately 70% office-based and 30% community-based work.
