Hours: Primary care hours are- Mon, Wed, Fri 8 am - 5 pm, Tue, Thu, 8 am - 7 pm, and one Saturday per month

DETAILS:
- 100% outpatient; Monday - Friday
- Patient-Centered Medical Home Model housed in a large multi-specialty center which includes Adult Medicine and Pediatrics as well as Endocrinology, Rheumatology and Dermatology
- Physician and APP Partnership exist to manage a panel of patients
- Call is 1:7 telephone only, and supported by nurse advisory line
