Additional Information: The Psychiatric Behavioral Health Nurse Practitioner will be responsible for providing treatment for patients with psychiatric illnesses in a primary care setting.
JOB DESCRIPTION:
- Responsibilities include evaluations and follow-up appointments for adult psychiatric patients requiring medication management.
- Provide counseling, brief therapy, psychoeducation about health/behavioral health and other care for patients as planned by the treatment team and support the treatment plan of the client.
- They will communicate with medical providers and work together in an integrated medical behavioral health model to improve the clinical outcomes for a population of patients with acute and chronic health and behavioral health conditions.
QUALIFICATIONS:
- Provider must have completed 4000 hours of clinical work. This mid-level should not require a collaborating psychiatrist to practice and be able to work autonomously in an FQHC setting
- Master's Degree
- Licensed in the state of DE
