Compensation: $275,000 - $300,000 plus bonus up to 20% of salary

Responsibilities:
- Partner with Market Operations Director to achieve operational and clinical excellence and world-class performance in quality, patient satisfaction, and clinician satisfaction, while reducing hospital admissions, and overall medical cost
- Provide primary care and see patients alongside clinical colleagues.
- Assist in recruiting clinical staff in the Chicago, Washington, and Louisiana geographies
Qualifications:
- IL or LA License and DEA required
- Currently reside in or willing to relocate to Chicago or Louisiana (Baton Rouge, Lafayette, or New Orleans)
- Experience in primary care is necessary, with value-based care preferred
- 3 years of clinical experience preferred
- Willing to travel up to 20% of the time
