Job Description

  • Hours: Flexible Start Time Between 7:30 AM - 8:30 AM 8 Hour Shifts | Alternative Schedule Available Working 9 Hours Monday - Thursday with Every Other Friday Off

  • Compensation: $52,468 - $95,360 Based on Experience

  • Additional Information:
    Requirements:
    - Bachelor's Degree in Nursing
    - Valid California license
    - 2 years of nursing experience working in the field of developmental disabilities
    - Fluency in Spanish is highly desirable
    - Mostly adult patient population; However the ability to see patients age 3 and up is required

    - This is a case management position that is mostly administrative responsibilities
    - Will provide a wide variety of monitoring, consultative and interdisciplinary team-related services, which promote optimal health and safety for individuals with a diagnosis of a developmental disability
    - Monitor consumers residing in specialized health care homes or community residential homes, making monthly, quarterly or annual visits to these residences to review consumer's health plans and ensure that appropriate health services are provided
    - May make home visits on a case-by-case basis to assess emergent situations
