Facility Details

Damian Family Care Centers, Inc. (DFCCs) is a well-established system of six primary care health centers located in Queens, Bronx, Manhattan and Ulster County New York. DFCCs assumed the operating certificate from Samaritan Health Services (SHS) in June 1999. All of our six clinical sites are licensed Article 28 Diagnostic and Treatment Centers (D&TC’s) and four of the sites are designated Federally Qualified Health Centers (FQHCs).

Since the initial 330(e) HRSA grant award for community health and homeless funding, DFCCs successfully obtained five-year continuation competitive funding both in 2006 and 2010; received Expanded Medical Capacity Funding award to increase homeless services in 2007; received Increase for Demand in Services (IDS) award in 2009 and Capital Improvement Project (CIP) funding in 2010. Our corporate offices are located in Briarwood New York.

Jamaica is a neighborhood in the New York City borough of Queens. It was settled under Dutch rule in 1656 in New Netherland as Rustdorp. Under British rule, the Village of Jamaica became the center of the "Town of Jamaica." Jamaica was the county seat of Queens County from the formation of the county in 1683 until March 7, 1788, when the town was reorganized by the state government and the county seat was moved to Mineola (now part of Nassau County).

When Queens was incorporated into the City of Greater New York in 1898, both the Town of Jamaica and the Village of Jamaica were dissolved, but the neighborhood of Jamaica regained its role as county seat. The neighborhood is part of Queens Community Board 12, which also includes Hollis, St. Albans, Springfield Gardens, Baisley Park, Rochdale Village, and South Jamaica.
