DETAILS:
- Scheduled Post-Discharge visits
- Provides primary care diagnosis and treatment in a family practice clinical setting to patients recently discharged from an inpatient or emergency department setting.
- Counsels and educates patients regarding treatment and refers to ancillary and support services.

- Will rotate between all of our primary care clinics (Coeur d'Alene, Post Falls, Rathdrum, and Kellogg)

QUALIFICATIONS:
- ID License REQUIRED
- DEA License REQUIRED
