Job Description

  • Additional Information: -A Float Position, will be rotating across our sites in Riverside and San Bernardino counties

    - Must have LCSW certification from the State of California

    - Responsible for providing individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses.

    PEDIATRIC POPULATIONS OF FOCUS:

    -Children experiencing homelessness.
    -Children who are high utilizers.
    -Children with Serious Emotional Disturbance (SED) or
    identified to be at Clinical High Risk (CHR) for Psychosis or
    experiencing a first episode of Psychosis.
    -Children enrolled in California Children's Services (CCS) / CCS
    Whole Child Model (WCM) with additional needs beyond the
    CCS qualifying condition.
    -Children involved in, or with a history of involvement in, child
    welfare (including foster care up to age 26).
    -Children transitioning from incarceration.
