Job Description

Hours: 40 Hours Per Week | Monday - Friday | Hours Between 8 AM - 5 PM | Limited Saturday Rotation (Approx. 4/Year) | May Consider Part-Time for 20-35 Hours/Week (with benefits)

DETAILS:
-Performs case management and counseling activities related to psychosocial needs, affecting the health and well-being of patients
- Assess the patient's psychosocial needs and determine appropriate treatment/referral needs
- Assists patients in accessing prescription cost assistance through indigent programs
- Acts as an advocate for patients when interacting with community agencies

QUALIFICATIONS:
-Licensed counselor/therapist in NY
-Seeking candidates who demonstrate a passion for outreach and community support and who empathize with the population we serve.
