Job Description

Hours: Monday - Friday 8 AM - 5 PM
Employed
Average patients seen: Case Management for 40-45 patients
Call Schedule: Rotate call once every 5 weeks
Sign-On Bonus : $2,500
Compensation: Hourly rate $23-$24/ per hour

Additional Information: - Valid LVN and drivers license required
- 2 years LVN experience
- Case Management for 40-45 patients
