$190, 000- $210, 000
*Sign-on bonus and retention bonus opportunities

Provide comprehensive family medicine and primary care services for infants, children and adolescents, adults and geriatric patients in accordance with current standards of care

Licensure and Certifications:
-Current and unrestricted Illinois medical license or ability to obtain
-Current and unrestricted DEA license
-Current CPR/BLS certification preferred
-Valid drivers's license
