Additional Information: Home Health administers skilled nursing care to patients on an intermittent basis in their place of residence. The RN provides this family-centered care to patients under the directive of overseeing Physician.
Qualifications:
- RN Oregon license
- AHA Basic Life Support
- Valid Oregon State Drivers License
- Coordinates care, services, and visits for the patient
- Documents ALL observations, interventions, and evaluations of care for each patient in compliance with agency policy and regulatory standards
- Reviews requests for services at the beginning of the shift and determines the efficient route, proper equipment needed, and timing of service
- Assesses the patient's condition and needs using critical thinking and clinical judgment: sets outcomes; implements appropriate nursing actions and collaborates with other disciplines to meet the patient's/family's physical, emotional, spiritual, social, and intellectual needs
