Ampla Health adopted the Patient-Centered Medical Home model of health care delivery that encourages improved quality and greater involvement of patients in their own care. The Medical Home is an innovative, team-based approach to providing health care services via a partnership between the patient, his or her primary clinician, and a healthcare team. The healthcare team may include primary care or specialty physicians, community-based providers, nurse practitioners, physician assistants, medical assistants, registered nurses, LVNs, medical assistants, etc. Together, following evidence-based guidelines for medical care, the team will offer consistent, coordinated care and communication, and arrange for specialty care to provide the best healthcare services possible for you.

Ampla Health is a 501 (c)3 non-profit network of community-based Federally Qualified Health Centers (FQHC) offering comprehensive medical, dental, mental health, and specialty healthcare services in Butte, Colusa, Glenn, Sutter, Tehama, and Yuba counties.

The City of Chico was founded in 1860 by General John Bidwell and incorporated in 1872. The City of Chico has grown to over 33 square miles with a population of 88,389 in the incorporated area and a greater urbanized area population of approximately 100,000. Chico is located in the Northern Sacramento Valley of California, ninety miles north of Sacramento on Highway 99, in Butte County, east of Interstate 5.

Chico is known as a well-managed city that values quality infrastructure and services, and maintains a special sense of community and small-town living as it has developed into a vibrant regional center for business, recreation, and cultural activities. There are also many recreational opportunities in and around Chico. Bidwell Park, one of the largest municipally owned parks in the nation (3,670 acres), is the focal point of the City's park system and offers numerous trails for biking, hiking, and equestrian use.

As a university community, Chico Performances hosts performances in theater and music, as well as lectures that typically are found in big cities. Sierra Nevada Brewery brings live concerts to Chico in its Big Room venue, and a number of small independent theaters offer performances throughout the year. 

Visitors often come to Chico for its easy access to outdoor recreation in biking, kayaking, fishing, and hiking. The city is a regional sports center in basketball, baseball, softball, swimming, and hockey.

Chico’s community spirit is seen in its downtown, which serves as a central gathering place for many community events such as farmers markets, Friday night concerts, and parades.
