As the Behavioral Health Therapist, you will identify, triage, and manage patients with behavioral health and substance use barriers within the primary care setting.
Assess patients with mental health, psychological, and/or substance use issues, create care plans, and manage care plans with a care team of primary care providers.
Conduct patient intakes, focused on diagnostic and functional evaluation, making appropriate mental health treatment recommendations to the primary care providers, and establishing positive rapport with patients.
Provide individual, family, and group counseling.
Provide episodic counseling services to patients in acute crisis.

Required Training & Experience:
Must possess a current CPR certification.
Must be licensed as an LMSW or LPC
CAADC is preferred but not necessary.
