Join us as a core member of the Collaborative Care Management team, working alongside the patient's medical provider, a consulting Psychiatric Consultant, and the broader primary care team. You will be responsible for supporting and coordinating the mental and physical health care of patients on an assigned caseload, in partnership with the patient's medical provider.
Required Experience:
Screening for common mental health and/or substance use disorders
Knowledge of the differential diagnosis of common mental health and/or substance use disorders
Evidence-based psychosocial treatments and brief behavioral interventions
Primary care, integrated care, or Collaborative Care Model settings a plus
Comfortable working with patients by telephone as well as in person
Open to:
