DETAILS:
The Social Worker coordinates social services to assist patients and their families in maintaining or improving their physical, mental, and psychosocial needs.
Provides direct social work assessments, problem identification, goal setting, and
implementation of care plans.

QUALIFICATIONS:
Bachelor's or Master's in Social Worker
Has an understanding of clinical settings and the ability to understand the patient population and mitigate deficits
