RESPONSIBILITIES:
Primary responsibility is to provide medical management to an established patient panel
Aid in continuity of care
Position is supported by Heritage Center for Behavioral and Mental Health, comprised of our behavioral health team of LCSWs, LMSWs, and psychiatric nurse practitioners
Flexible schedule Monday-Sat with earned admin time that has the potential to be completed remotely
See a diverse patient panel primarily consisting of adults
Part-time and full-time scheduling available. We will strongly consider experienced providers seeking a flexible schedule or balance alongside another job

QUALIFICATIONS:
NY state-licensed
BE/BC
Willing to commute to the site clinic location and integrate amongst the patient panel to provide care
