DETAILS:
Provide support to teams to ensure timely review of labs, diagnostic imaging, and documents for patients
between visits
Provide in-person or telehealth urgent care on a regular schedule or as needed
Be available to primary care teams at all sites as a backup for an urgent medication review or refill

QUALIFICATIONS:
CA License REQUIRED
DEA License REQUIRED
3 years of community health experience PREFERRED
