Finding the need to provide continuity of care in several areas of concern, Family & Medical Counseling Service has developed a position that lends itself to someone developing a skill set in our primary care and behavioral health programs.
The RN coordinator will serve as a resource when medical attention is needed and assist with coordinating medical care with behavioral health care for patients who need both.
The provider must also be willing to:
Act as a patient advocate, protecting privacy and confidentiality issues.
Establish a plan of care with patients, providers, and payers that identifies and continuously reassesses cost-efficient, appropriate levels of care
Maintain a comprehensive working knowledge of community resources, payer requirements, and network services for the target population.
QUALIFICATIONS:
RN Degree
Familiar with EHR
Licensed in D.C. Licensing in MD also beneficial
BLS certified
