QUALIFICATIONS:
1-2 years previous clinical nursing experience preferred
NH RN License
BLS, ACLS required

Coordinate team-based care to provide health services to individuals, through effective partnerships with patients, their caregivers/families, community resources, and their physician. Facilitates a "shared goal model" within and across settings to achieve coordinated high-quality care that is patient and family-centered.

Assess patient before and post pain procedures
Obtain vitals and provide accurate patient assessments
Be able to provide triage support for patients within the practice
Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support specialists
Responsible for daily assignments, monitoring staff needs, management of acute needs of office staff
MA support
Assist with surgical and procedure scheduling and coordination
