Both positions involve the following:
Obtains and documents health history, information on chief complaint, vital signs, and health risk factors, and assesses disease conditions.

Nurse Care Coordinator: Responsible for highrisk & chronically ill patients, serving as a point of contact, advocating for preventative care and reducing ER utilization. 1 yr. min experience.

QUALIFICATIONS:
Both RN & NCC roles require a DE state license. Will accept new grads for RN roles.
