Home Health administers skilled nursing care to patients on an intermittent basis in their place of residence. The RN provides this familycentered care to patients under the directive of overseeing Physician.

Qualifications:
RN Oregon license
AHA Basic Life Support
Valid Oregon State Drivers License

Coordinates care, services, and visits for the patient
Documents ALL observations, interventions, and evaluations of care for each patient in compliance with agency policy and regulatory standards
Reviews requests for services at the beginning of the shift and determines the efficient route, proper equipment needed, and timing of service
Assesses the patient's condition and needs using critical thinking and clinical judgment: sets outcomes; implements appropriate nursing actions and collaborates with other disciplines to meet the patient's/family's physical, emotional, spiritual, social, and intellectual needs
