JOB DETAILS:
Passion and excitement for a unique and challenging primary healthcare service
Clinically adept, with the ability to communicate and educate a diverse patient population
Experienced and comfortable treating chronic conditions such as COPD, hypertension, diabetes, etc
Demonstrated patience to develop a progress plan and assist patients with their goals
Give access to specialty care services, with the goal of alleviating the need for escalation to hospital services
Possesses the ability to work autonomously and independently, providing support to both patients and support staff alike

ADDITIONAL REQUIREMENTS:
BE/BC
DEA, CDS
State licensed: Will consider applicants from the DMV area, assisting with licensing cost reimbursement
1 year experience required, 2 years preferred in a primary care/outpatient setting
