Compensation based on years of experience and the ability to effectively perform job functions. Starting salary from $120k annually.

JOB DETAILS:
Passion and excitement for a unique and challenging primary healthcare service
Clinically adept, with the ability to communicate and educate a diverse patient population
Experienced and comfortable treating chronic conditions such as COPD, hypertension, diabetes, etc
Demonstrated patience to develop a progress plan and assist patients with their goals.
Give access to specialty care services, to alleviate the need for escalation to hospital services
Possesses the ability to work autonomously, providing support to both patients and support staff alike

ADDITIONAL REQUIREMENTS:
BE/BC, State licensed: Will consider applicants from the DMV area
1 year experience required, 2 years preferred in a primary care setting in within a clinical environment, managing chronic conditions.
Candidates must have demonstrated the ability to provide assessments, provide thorough screening, and assist with lifetime support
