As an expansion of Summit Health's Primary Care Team, we are seeking a Family Medicine Nurse Practioner to care for our growing population. This position will not host a traditional panel but will truly act as an extension of our Primary Care Providers.


Responsibilities will include:

Seeing Same-day patients
Follow up visits
Patient education
Assisting with In-Basket Messages
Being the key link to delivering quality care supporting our open access model


Qualifications:
Current OR State APN License
Minimum 1-years' experience in Primary Care
