Call Schedule:

The Nurse Practitioner is an integral part of the clinical care team providing primary care services to the patients and families served by PCHC. Essential Duties:
Provides comprehensive, high quality and cost-effective primary care services to a diverse population in a patient and family centered environment.
Within the course of a primary care visit prescribe appropriate medications, order necessary screening and diagnostic tests, and confers and refers for specialty care, when necessary.
Performs health assessments and physical examinations for adults and children, develops patient plans and works with patients/families to develop self-management goals.
Maintains the safety and security of the patient/family's Personal Health Information and is compliant with HIPAA standards.
Participates in Quality Improvement Initiatives and provides input on the development of policies and procedures which impact the health delivery service.
