The primary responsibilities of the position is to provide safe, quality and culturally sensitive medical services to our patients who seek services. Provider shall initiate and respond to coordination of care activities on behalf of patients with interdisciplinary care team members, including psychiatrists, nursing staff, health navigators, primary care/behavioral health/substance use providers, case managers, and other healthcare personnel to develop, implement, or evaluate treatment plans.
QUALIFICATIONS:
Colorado licensed Nurse Practitioner with experience working in a clinic setting
Bilingual skills (English/Spanish) desirable.
Fully on-site ( no remote work/telemedicine)
